How to Become a DMEPOS Supplier
The steps to becoming an accredited DMEPOS supplier and enrolling for Medicare reimbursement can feel complicated. This step-by-step guide is meant to simplify the enrollment process and make it easier for you to navigate.
What Is a DMEPOS Supplier?
DMEPOS suppliers distribute durable medical equipment, prosthetics, orthodontics and supplies to physicians and medical professionals. All suppliers must get accredited through a CMS-approved organization to become eligible for Medicare reimbursement.
How to Enroll as a DMEPOS Supplier
As an accredited DMEPOS supplier, you can be reimbursed by Medicare for 80% of the total costs of the items or services provided. The beneficiary is responsible for the other 20%. Follow the five step guide below to start your DMEPOS accreditation and Medicare enrollment today.

Step 1: Apply for accreditation
Find a Centers for Medicare & Medicaid Services (CMS) approved third-party accreditation organization to verify that your business meets their requirements. Once you select the organization, fill out the application and upload all relevant documents. Processes vary between organizations, so pay close attention to the site's instructions.
Applications can take 4-6 months to be fully reviewed. You can move forward with the next steps in the meantime; however, we recommend waiting for accreditation approval before moving forward in case you are denied. Once approved, the accreditation organization will conduct an unannounced site visit to ensure that you meet the DMEPOS quality standards.
Step 2: Get an NPI Number
Once your business has been approved, get a National Provider Identifier (NPI) number. Every individual practice location needs a unique NPI.
This step can be skipped if you already have an NPI number for each location. If you don’t, you can get one through the National Plan & Provider Enumeration System (NPPES) website.
If you don’t remember your NPI number, or are unsure if you have one, search the NPI registry.
Step 3: Complete the Enrollment Application and EFT Authorization
Next, fill out the enrollment application and electronic funds transfer (EFT) agreement through PECOS, the CMS’s online registration system.
Register to become a PECOS user and follow the tutorials to correctly fill out the enrollment and agreement forms. You will be assigned an enrollment contractor. Find yours to track your application status.
Step 4: Pay the application fee
After submitting your enrollment application, pay the application fee on the PECOS payment page.
The fee amount changes annually, so be sure to check the page for the official amount.
Step 5: Purchase surety bond

Once you have completed all of these steps, you will be successfully enrolled in the Medicare program and eligible for DMEPOS billing reimbursement.
How to Renew Your DMEPOS Enrollment
You are required to revalidate your DMEPOS supplier Medicare enrollment every three years through the PECOS system. All of your information will still be uploaded from your initial enrollment. Just follow these simple steps:
- Review and update your information
- Upload any new supporting documents
- Sign your revalidation forms
You can revalidate your accreditation three months before the expiration date. If you receive a revalidation notice before that date, you can start the renewal process early.
CMS does not grant extensions, so make sure you revalidate your application in time. If you miss the deadline, your reimbursement privileges will be put on hold and your enrollment may be deactivated.
How Much Does It Cost to Become an Accredited DMEPOS Supplier?
In 2025, the Medicare provider enrollment application fee was $730. However, fees vary each year. Check the PECOS website to find the current enrollment cost.
